Uniform Certification Application 

Applicant firms interested in seeking certification as both a Minority Business Enterprise (MBE) and Disadvantaged Business Enterprise (DBE) simultaneously should use the link below to complete the Uniform Certification Application. We recommend that you save the form to your desktop as you work on completing the entire application. If an area of the application does not apply to you or your firm, note “Not Applicable” or “Does Not Apply.” Do not leave blanks. Blanks are interpreted as incomplete information. Applicants who submit an incomplete application will delay the certification process. Sign and notarize the Affidavit of Certification for each minority/disadvantaged owner.
